United States Visa Lottery Form 2023/2024 – How to apply USA Green Card Lottery

America Visa Lottery/green card Application – This is to informall interest individuals, this is to inform you that the American Visa Lottery 2023/2024 application form is now available.

Application for the American Visa Lottery is should be done on their official portal/website of the State Department.  Any Nigerian can now apply for it.

To know more about the lottery is that the online form is called DS-5501, the form is only available during the DV open registration period.

1. FULL NAME– Last/Family Name in Capital letters, First/Given Name, Middle Name
2. DATE OF BIRTH
3. GENDER
4. CITY WHERE YOU WERE BORN
5. COUNTRY WHERE YOU WERE BORN
6. COUNTRY OF ELIGIBILITY

3. PHONE NUMBER – Please use a valid and working mobile number
4. E-MAIL ADDRESS –Put an active email address.
5. DEGREES ACQUIRED – Indicate which one of the following represents your own highest level of educational achievement:

• Primary school only,
• High school, no degree,
• High school degree,
• Vocational school,
• Some university courses,
• University degree,
• Some graduate level courses,
• Master degree,
• Some doctorate level courses,
• Doctorate degree

3. SPOUSE INFORMATION – Name, Date of Birth, Gender, City/Town of Birth, Country of Birth, and Photograph.

Once you successfully register the entry, you will see a confirmation screen that contains your name and a unique confirmation number. Print this confirmation screen for your records. You will need this confirmation number in order to check your status.

On submission of your application form, ensure you print out your confirmation slip and KEEP IT SAFE, for future purpose.

All application should be completed and submitted on or before November,

• Application is free of charge and your are not to pay any money to any individual or group of persons
• You can check your application status, from the month of March.
• Never send documents (birth certificate, work experience, school records etc.) anywhere, other than making your application an online entry.
• Incomplete application will be disqualified.
• Multiple entries will be automatically disqualified.
• False information will be disqualified, so ensure all the information given is correct.
